Dates of Coverage
Coverage for individual participants begins on the date of employment with UT Health or on the issue date of Texas license or training permit, whichever occurs latest. Coverage ceases for events occurring after the date of employment or after registration with The University of Texas System is terminated.
Credentialing Requests to The University of Texas System
You may download proof of current coverage from New Innovations. It is found on the “File & Notes” tab of the personnel profile.
To obtain a written claims history for a past physician or dentist, The University of Texas System requires a written request signed by the requesting doctor. Please be sure to state that you authorize The University of Texas System to release your medical liability claims history. These requests should be sent to The University of Texas System via email to: HLC@utsystem.edu.
Requests for residents and fellows on rotation at Memorial Hermann Hospital System prior to July 1st of 2019 should also be sent to Memorial Hermann’s Office of Insurance Services by email to insverify@memorialhermann.org.
