Common Program Requirements Effective July 1, 2016 – Fellow Eligibility Exception Language

III.A.2.b) A Review Committee may grant the following exception to the fellowship eligibility requirements:

An ACGME-accredited fellowship program may accept an exceptionally qualified applicant**, who does not satisfy the eligibility requirements listed in Sections III.A.2. and III.A.2.a), but who does meet all of the following additional qualifications and conditions: (Core)

For exceptional candidate approval, the “Exceptional Candidate Review Checklist” along with the documents listed below to be sent to the GMEC office for review (email: ms.gme@uth.tmc.edu). The “Exceptional Candidate Review Checklist” is found in the New Innovations Intranet under Resources > _REQUEST FORMS

• GME Office – Exceptional Candidate Form
• Letter of support from Program Director
• Candidate CV
  • ACGME-i Programs meet the approval standards.
• Candidate Letter Interest
• Letter(s) of Recommendation
• Visa status/ ECFMG certificate (if applicable)
• Any additional supporting documents
• Include, Program requirements per Specialty
