We understand that patients may often feel overwhelmed during a doctor visit, but we want each patient to fully understand all aspects of his or her treatment. Thus, we fully endorse the Ask Me 3 initiative.

Patient Forms

Questionnaires

We ask that all of patients complete standardized questionnaires that summarize current symptoms, medications, previous treatments and other relevant information. Those questionnaires are posted here.

Completing the appropriate questionnaire before your office visit will greatly expedite your care. If you cannot complete the questionnaire before your office visit, please arrive at least 30 minutes (for new patients) or 15 minutes (for established patients) before your scheduled visit time.

At the time of your visit, you will also need to verify insurance and sign various disclosures as required by current law and best practices.

Note: Each questionnaire is designed for a specific type of office visit. Please be sure to complete the appropriate questionnaire for your visit.
