Alumni Requests
Verification of Training
- GME verifies dates of training only as listed on appointment agreements.
- Requests must be submitted in writing by the requesting entity with a signed release from the former resident/fellow.
- GME will not provide any verbal verifications.
- State Medical Boards: Submit the official form with a signed release from the resident/fellow. These requests are completed and signed by the Program Director.
- Request can be submitted to [email protected].
Replacement Certificates
- Requests must be made through your former program and forwarded to GME.
- For name changes, provide:
- Legal proof of name change (marriage certificate, divorce decree, court order)
- Original certificate (will be destroyed upon reissue).
Other Requests
- Change of Address → Email updated info to [email protected]
- PSLF Forms → Should be routed to the Program Coordinator
- Previous Documentation (case logs, etc.) → Contact the Program Coordinator
