Alumni Requests

Verification of Training

  • GME verifies dates of training only as listed on appointment agreements.
  • Requests must be submitted in writing by the requesting entity with a signed release from the former resident/fellow.
  • GME will not provide any verbal verifications.
  • State Medical Boards: Submit the official form with a signed release from the resident/fellow. These requests are completed and signed by the Program Director.
  • Request can be submitted to [email protected].

Replacement Certificates

  • Requests must be made through your former program and forwarded to GME.
  • For name changes, provide:
    • Legal proof of name change (marriage certificate, divorce decree, court order)
    • Original certificate (will be destroyed upon reissue).

Other Requests

  • Change of Address → Email updated info to [email protected]
  • PSLF Forms → Should be routed to the Program Coordinator
  • Previous Documentation (case logs, etc.) → Contact the Program Coordinator
  • W-2 → Contact Payroll for more information
