Your safety, wellbeing, and ability to report concerns confidentially are priorities of the GME Office. Use the resources below to report concerns, safety issues, duty hour violations, or injuries.

Anonymous Report of Resident Concerns & Duty Hour Violations

Submit concerns anonymously. Reports are reviewed by the GME Office. A non-retaliation policy applies.

Fatigued Resident Travel Reimbursement

If you are too fatigued to safely drive home, you may request reimbursement for ride share services via your program coordinator in accordance with the GMEC approved policy.

Title IX Reporting & Support

Title IX protects residents and fellows from discrimination, sexual harassment, sexual misconduct, and gender-based violence.

Residents may report incidents or seek confidential guidance through the UTHealth Title IX Office.

Injury & Exposure Reporting

General Injury Reporting

For workplace injuries, accidents, or safety incidents:

UTHealth Residents/Fellows
Use the Supervisors’ First Report of Injury.

Military Residents/Fellows

  • File claims through your respective service branch.
  • If incident occurred on UTHealth property, complete the Military Resident Incident Report Form.

Visitors/Guests
Use the Visitors/Guest Accident Investigation Form.

Needle Stick, Bloodborne Pathogen, or TB Exposure (URGENT)

If you experience a needle stick, bloodborne pathogen exposure, or possible TB exposure, take immediate action:

Step 1: Apply First Aid Immediately

  • Clean exposed skin with soap and water for at least 15 minutes
  • Flush eyes or mucous membranes with water or saline for at least 15 minutes
  • If source patient is known, keep individual onsite for blood draw
