Memorial Hermann-Texas Medical Center
Founded in 1925, Memorial Hermann-Texas Medical Center (TMC) is the primary teaching hospital for the McGovern Medical School at The University of Texas Health Science Center at Houston (UTHealth Houston). Memorial Hermann-TMC provides leading-edge care in heart, neuroscience, orthopedics, women’s health, general surgery, organ transplantation, and much more. As one of only two certified Level I trauma centers in the greater Houston area, this 1104-bed hospital provides 24/7 emergency and trauma care. Memorial Hermann Life Flight® provides emergency rescue within a 150-mile radius. The Memorial Hermann Medical Plaza, a 30-story medical office building, supports the Institute and other services with additional physician office space, an Ambulatory Care Center, and two floors dedicated to retail shops, services and restaurants.
Lyndon B. Johnson Hospital
Part of the Harris county hospital system, Lyndon B. Johnson Hospital is a 215 licensed-bed acute care hospital offering a full range of medical services. LBJ Hospital is a verified Level III trauma center and was the first such center designated in Texas. It is also the state’s busiest Level III trauma center, with more than 80,000 emergency patient visits yearly. LBJ Hospital carries the distinction of being a regional center for neonatal intensive care for high-risk deliveries and very low birth weight infants and provides an array of specialized medical and surgical services. LBJ Hospital also offers rehabilitation services.
Harris County Psychiatric Center (HCPC)
The Harris County Psychiatric Center is home to the inpatient psychiatric ward, including dedicated Geriatric Psychiatry care by Geropsychiatrists and nurse practitioners.
UTHealth Houston Professional Building
Numerous specialty rotations are housed in the heart of the Texas Medical Center in the Professional Building, offer both practitioners and patients easy and comprehensive access to high quality care. Some of the offered specialties include Movement Disorder – Neurology, Urology, Gynecology, and more.
Behavioral and Biomedical Sciences Building
The Behavioral and Biomedical Sciences building, located near the Cooley University Life Center and the UTHealth Houston Dental School, offers services in Neurology and Psychology.
Nursing Home (NH)
Highland Park Nursing home is a unique experience for trainees to gain valuable experience in medical directorship, long-term care management, short-term skilled nursing facility care, and also hospice care within a facility. Fellows spend both dedicated rotation time and have monthly longitudinal experiences within Highland Park Nursing home, spending time with a dedicated physician and nurse practitioner.
LBJ House Calls
The LBJ house call program provides routine medical care to patients who are immobile or at risk for frequent hospitalizations due to their chronic disease. Providing comprehensive medical and social services to older adults unable to come to the clinic, this program includes Geriatricians, Hospice and Palliative Medicine physicians, geriatric nurse practitioners, as well as physical therapists, social workers, dieticians, and pharmacists. The program offers coordinated care of a multidisciplinary treatment plan with home visit physicians and nurse practitioners becoming the primary care physician for these patients. With frequent home visits, we strive to prevent hospitalizations and maintain a healthy quality of life at home. Primary care, Geriatric care, and Palliative care are all available on an in-home basis. Medical services include but are not limited to a complete geriatric assessment, screening, and treatment for common geriatric illnesses such as dementia, diabetes, heart problems, osteoporosis, arthritis, functional decline/falls, and malnutrition, and evaluation and management of medications.
Settegast Community Health Center
The Settegast Community Health Clinic is located in northeast Houston on North Wayside Drive, just south of the intersection with Tidwell Drive. Settegast is part of Harris Health System, and proudly provides comprehensive assessment and primary care by Geriatric Medicine trained physicians and nurse practitioners. In addition to consultations for specific Geriatric concerns, preventative care and annual physical exams, chronic and acute health care management, and memory, mood, medication, mobility, and functional assessments are provided to help patients achieve what matters most to them.
Center for Healthy Aging
The Center for Healthy Aging – Bellaire is located in the UT Physicians building near the intersection of 610 West Loop South and Bellaire Blvd. A part of UT Physicians, Geriatric Medicine physicians, nurse practitioners, nurses, and social workers collaborate to improve outcomes for older adults. Focusing on helping older adults age in healthy and functional ways, generalist and specialist care is provided under one roof at this UT Physicians Center. In addition to consultations for specific Geriatric concerns, preventative care and annual physical exams, chronic and acute health care management, and memory, mood, medication, mobility, and functional assessments are provided to help patients achieve what matters most to them.
