What to Bring to Your Appointment
If it is your first time visiting our clinic, please fill out the online Patient Portal before you arrive. Bring the following items to your appointment, in addition to anything requested by your specific provider team:
- Any relevant medical records
- Your insurance card(s)
- Your photo ID
All patients must complete the required forms by logging in before their first appointment. Patients who do not complete the forms prior to their appointment may be rescheduled to avoid delays in care, so please try to complete these beforehand.

Our group includes nurse practitioners, physician assistants, medical assistants, and many others who will help with your care. You will likely see multiple caregivers, and we know that you will enjoy working with everyone here.
At some clinics in our group, we may also have a resident or medical student present for patient visits. This is a part of our educational program at McGovern Medical School at UTHealth, and is important to our mission of training the neuroscience specialists of the future. Please keep in mind that the involvement of trainees can enhance patient care, and it helps the teacher remain up to date.
