Imaging innovation: OCTA study suggests earlier detection of hypertensive retinopathy
A new study by UTHealth Houston ophthalmologist Rabia Karani, MD, MPH, offers compelling insight into the clinical potential of optical coherence tomography angiography (OCTA) for detecting hypertensive retinopathy earlier and more reliably than traditional methods.
Hypertensive retinopathy is typically diagnosed via clinical examination. Although fluorescein angiography remains the gold standard for retinal vascular imaging, it’s invasive and not always ideal for routine monitoring. Karani’s research explored how OCTA, a noninvasive imaging technique, might serve as a complementary or even earlier diagnostic tool, particularly for subtle vascular changes associated with hypertension.
“OCTA allows us to evaluate the retinal vasculature without using dye,” Karani says. “Our study showed that arteriolar tortuosity — one of the early signs of hypertensive damage — could be detected earlier using OCTA compared to clinical observation alone.”
Importantly, the study also found that OCTA had stronger observer agreement than clinical examination. “That reliability makes it a valuable adjunct, especially in a busy clinical setting where consistency across providers is crucial,” she adds.
The implications go beyond the eye. Because the condition reflects systemic vascular health, earlier identification of hypertensive retinopathy could signal more aggressive blood pressure management, or even prevent complications like retinal vascular occlusions or macroaneurysms.
While OCTA technology is not yet widely available, Karani notes that some private practices already use it routinely. “It’s easier for technicians to operate than fluorescein angiography, and as it becomes more accessible, I think we’ll continue finding new clinical applications,” she says.
