Trauma Service PT/OT Consultation

Physical therapy (PT) and occupational therapy (OT) are both vitally important to the care of our trauma patients. While there may be overlap in their therapies, they each serve a unique role in the post-injury rehabilitation process.

Physical Therapy / Occupational Therapy Focuses on improving mobility and function while reducing pain following injury
Focuses on helping to regain the ability and independence to complete activities of daily living

PT and OT consultation are not essential for every trauma patient and should be based on the specific needs of each patient. Please remember that both PT and OT are finite resources and should be used thoughtfully on the trauma service to ensure they can focus their efforts on the patients who will derive benefit from their expertise.

General recommendations for consulting PT and OT on the trauma service:

Combined PT & OT Consultation Recommended

  • Age >65
  • Admitted for ground level fall
  • Weight >250lbs (113kg)
  • Cognitive impairment/traumatic brain injury
  • Frailty
    • Trouble getting out of bed/chair independently
    • Trouble dressing, bathing, shopping, cooking, etc.
  • Disposition to skilled nursing facility (SNF) not for rehab only
  • Disposition to acute inpatient rehab (AIR)

PT Consultation Recommended

  • Lower extremity injuries (function limiting)
  • Spine fracture/injury
  • Disposition to SNF for rehab only

OT Consultation Recommended

  • Upper extremity injuries (function limiting)
  • Cervical spine injuries with sensory and/or motor deficits

*** For patient’s not meeting the above criteria, exceptions may be made for PT/OT consultation on a case-by-case basis based on clinical concerns.

PT & OT consultation are not recommended for the following patients:

  • Bed bound at baseline
  • Following laparotomy who are mobilizing with nursing
  • Hospital re-admission with no new injuries or status changes
  • Patients who are intubated and unable to tolerate sedation holidays for assessment/therapy
