  • Submission of receipts that are sixty (60) days old and later should be covered by a 60-day Late Justification Memo.
  • Qualtrics Memo forms should be approved and submitted in PDF format by your Program Coordinator

    2. Receipt,
    3. Self Attestation Form
    4. 60-day Late Justification Memo, (if applicable)

  • All Medical Residents/Fellows paid thru UTHealth should continue to use the Supervisors’ First Report of Injury .
    • NOTE: Military trainees should file their claims directly thru their respective service branches.
  • All Military Residents/Fellows who need to report an incident taking place on UT Health property should use this form.

Verification of malpractice coverage and claims history

• UT System residents/clinical fellows, are covered under two separate malpractice insurance policies – The University of Texas System & Memorial Hermann Hospital.
• Memorial Hermann Hospital maintains a policy for residents/clinical fellows rotating within their institution and UT System maintains a self-insured policy for all other locations. To get a complete verification of your past coverage and claims history, residents/clinical fellows must contact both entities.
