Thank you for scheduling an appointment at UTHealth Neurosciences. This page contains information to help you prepare for your upcoming appointment at the clinic located within the UTHealth Houston Behavioral and Biomedical Sciences Building, 1941 East Road, Suite 4358.
We request that you bring the following to your appointment:
- Photo ID
- Insurance and pharmacy card(s)
- Any medical records, especially any imaging CDs and past neuropsychological reports
Directions
UTHealth Neurosciences is located in the Texas Medical Center. The clinic is on the 4th floor at 1941 East Road, Suite 4358.
Convenient self-parking is available in the building’s parking lot, located across East Road from our clinic. Self-parking is $10 per day.
Once you park, cross the street to 1941 East Road; our building is next to the UTHealth School of Dentistry. Once in our building, take the elevator to the 4th floor.

New Patient Registration
Please help us provide the best possible care by completing the New Patient Questionnaire in the MyUTHealth Patient Portal before your appointment. It should only take a few minutes of your time, and will help our clinicians provide you with the best care possible.
To complete the questionnaire, you will need a list of your current medications. Please be sure to include the name, dose, and instructions for taking.
You should have received an invitation to sign up for the MyUTHealth Patient Portal at the email address you provided during the time of registration. This secure site is where you will input your personal information and your medical history, so our clinicians can provide you with the best care possible. It is also where we communicate with our patients. Here, you can send a message to your provider, check lab results, request refills, and more.
We also request that you bring any medical records you may have to your appointment, along with your insurance card(s) and photo ID.
