Prepare For Your Appointment: Stroke Care at the UT Professional Building
Thank you for scheduling an appointment at UTHealth Neurosciences. This message contains information to help you prepare for your upcoming appointment.
We request that you bring the following to your appointment:
- Photo ID
- Insurance and pharmacy card(s)
- Any medical records
Also, if you are a new patient, please complete the New Patient Questionnaire online before your appointment, whether it’s an in-person or a virtual visit.
New Patient Registration
Please help us provide the best possible care by completing the New Patient Questionnaire at the link below. It should only take a few minutes of your time, and will help our clinicians provide you with the best care possible.
To complete the questionnaire, you’ll need a list of your current medications. Please be sure to include the name, dose, and instructions for taking.

Directions
UTHealth Neurosciences is located in the Texas Medical Center. The clinic is on the 10th floor at 6410 Fannin Street in Suite 1014.
Convenient self-parking is available in the building’s parking garage, called the University of Texas Professional Building Parking Garage. Located at 6414 Fannin Street, it is accessible from both Fannin Street and Main Street. Self-parking is $10 for under 6 hours, and $20 for over 6 hours/one day.
Valet parking is also available. To utilize valet parking, turn right into the second garage entrance on Main Street. Valet is currently $20/day.
Once you park, ride the elevator to the first floor of the garage, then walk across the breezeway to UTPB (University of Texas Professional Building). Then, take the elevator to the 10th floor.
