Applicant Eligibility
Applicants must:
- Have successfully completed USMLE or COMLEX Step 1, 2, and 3.
- Have completed an ACGME-accredited residency or fellowship (or Canadian equivalent) before the fellowship start date.
- Be eligible for a Texas Medical Board license by May 2027.
- Be eligible for a Texas DEA registration by July 2027.
Required Application Materials
Applicants should submit:
- Fellowship application
- Personal statement (one page)
- Three letters of recommendation (including one from the Program Director)
- Current curriculum vitae
- In-Training Examination results
- Medical licenses and certifications
- National examination results (if applicable)
- USMLE/COMLEX scores
- Professional headshot
- Government-issued photo identification
