Acres Home Community Health Center (CHC)
Based at the Acres Homes Multi-Service Center on West Montgomery Road, the Acres Home Partnership was established in 1996 to provide community health services and education to area residents through an innovative community-driven health promotion program involving local leaders and mentors. The partnership’s home visit programs also provide a rewarding learning experience for students outside of the classroom. Acres Home CHC offers a class in elder health to our nursing students, which provides the opportunity to establish personal friendships with the residents where they live for three months. The students learn now to identify needs and find educational and community resources for the patients.

Aldine Health Center
The Aldine Health Center is a Family Medicine resident training site. Built in 1996, it was specifically designed as a teaching site and is currently our largest clinic with 44 exam rooms and an in-house pharmacy. Similar to Acres Home, residents are responsible for their own panel of patients from a primarily indigent patient population. Family Practice faculty attending are always available for clinical consultation. An up-to-date Family Medicine library provides current reference materials as well as on-line computer search capabilities. Residents choose the Aldine CHC as their continuity clinic on a volunteer basis.

Baytown Health Center
The Baytown Health Center serves the primary care needs of Harris County residents living in the Baytown area. Baytown CHC provides health care services for more than 50,000 patient visits annually. In addition to regular adult and pediatric services, Baytown CHC provides ophthalmology, obstetrical/gynecology, podiatry, pharmacy, laboratory, radiology, psychiatry services and behavior counseling.

Settegast Health Center
The Settegast Health Center opened in Northeast Houston in 1967. In addition to adult and pediatric services, Settegast Health Center provides dental, podiatry, optometry, radiology, laboratory, pharmacy services, psychiatry and counseling, as well as a variety of nutrition, health education and social services.
