Diabetes Care and Education Specialists

We are committed to providing education, support, and resources to people living with diabetes, to increase understanding and improve management of this condition. Diabetes care and education is a collaborative process through which people with or at risk for diabetes gain the knowledge and skills needed to successfully manage the disease and its related conditions. It can take place in a group or one-on-one setting as part of a recognized or accredited education program or in an informal format.

Our Diabetes Care Team promotes self-management to achieve individualized behavioral and treatment goals that optimize health outcomes. Our Certified Diabetes Care and Education Specialists (CDCES) are core health professionals who possess comprehensive knowledge of and experience in diabetes prevention, prediabetes, and diabetes care and management.

Together with registered dietitians (RD), a diabetes community care specialist, and our diabetes research assistant and coordinators, the team provides patient education and support that is in compliance with the Association of Diabetes Care and Education Specialists’ Diabetes Education and Accreditation Program (DEAP). The program has been nationally accredited for the past 9 years.
