UT Physicians Primary Care Team earns Patient-Centered Medical Home Recognition

The UT Physicians Primary Care Team has earned their recertification as a Patient-Centered Medical Home (PCMH) for the fourth year running. This prestigious honor issued by the National Committee for Quality Assurance (NCQA) focuses on identifying physician practices that put patients at the forefront of care. This is an arduous process that requires ongoing participation from everyone, but well worth it.

“Every year we have to meet several standards in patient care, access and continuity, care management and support, care coordination and care transitions, performance measure and quality improvement. This is definitely a team effort. Our providers, staff and management team work together throughout the entire year to make sure that we are meeting the metrics and educating staff and providers on workflows to adjust when needed,” said UT Physicians Director of Clinical Operations Maria Villalobos-Cruz.
