We strive to provide quality comprehensive services to our new and current patients. As part of this service, we are requesting you complete the Patient Profile form. This will help the pharmacists assist your physicians and nurses at the Hemophilia Treatment Center to provide the proper treatment.
Please let the pharmacy know when your current medications or allergies change by updating the Patient Profile form. You can download the Patient Profile form, and when completed you may mail it, fax it to the pharmacy at 713-500-8384.
