How case managers support participants

Houston-HVIP provides provides individualized case-management services to support participants in achieving their recovery and long-term wellbeing goals following a violent injury. Case managers work collaboratively with each participant to develop a plan of care tailored to their specific needs.
General case management includes:
- Identifying and researching relevant programs and community-based organizations aligned with the participant’s needs and goals
- Facilitating referrals and connections to services such as housing assistance, employment support, and education, among others
- Advocating on behalf of participants when engaging with service providers
- Providing emotional support throughout the recovery process
Each participant’s journey is unique, and case managers are committed to offering compassionate, personalized support. The list below outlines the range of services that may be available based on the participant’s circumstances and preferences.
The Houston-HVIP can connect eligible clients to resources to support immediate and long-term housing needs.
- Discuss housing needs and research potential housing programs for the client
- Provide information about housing-related resources
- Refer eligible clients to housing assistance programs
- Assist client in completing applications for rental and housing programs
- Assist in compiling necessary documents (ID, income verification, etc.)
- Assist with finding emergency relocation services
- Help client complete Coordinated Entry assessment
- Support communication with housing authorities or landlords
The Houston-HVIP can help support economic needs during the recovery journey by assisting with the process of finding, applying for, and utilizing eligible public assistance benefits.
- Discuss needs, eligibility, and assist with application to public benefits supports programs such as:
- Supplemental security income (SSI)
- Supplemental Nutrition Assistance Program (SNAP)
- Temporary Assistance for Needy Families (TANF)
- Special Supplemental Nutrition Program for Women, Infants, and Children (WIC)
- Crime Victims Compensation
- Medicaid
- Children’s Health Insurance Program (CHIP)
- Unemployment Insurance
- Earned Income Tax Credit
- Utility assistance
- Provide support with completing income-verification documents
- Assist client with financial coaching or budgeting
- Refer client to agencies that offer financial coaching or budgeting tools
The Houston-HVIP can connect eligible clients to resources to support their day-to-day nutritional needs.
- Provide information about local food resources
- Connect eligible client with local food pantries and meal programs
- Assist with completing SNAP applications
- Provide education on food budgeting or grocery assistance apps
- Assist with scheduling food deliveries (if available/eligible through local programs)
- Assist in finding community food-distribution events
The Houston-HVIP helps connect clients to appropriate medical care for their injury-related and ongoing medical needs.
- Assist eligible client in applying for sliding scale clinics or county health systems
- Assist with insurance-related applications, including Medicaid/CHIP or ACA health insurance enrollment
- Help client research, find, and schedule for general, dental, vision, or specialized care
- Coordinate with medical care team to support physical recovery
- Provide information on clinics with free STD or pregnancy testing
The Houston-HVIP provides support and treatment services to address clients’ mental health and substance use needs.
- Discuss emotional wellness and coping strategies with client
- Refer eligible clients to outpatient or residential mental health services
- Discuss therapy options with client
- Identify and make referrals to insurance-covered therapy providers
- Assist with scheduling therapy on behalf of client
- Assist in contacting substance-use recovery or detox programs
- Help with referrals and/or scheduling intakes or assessments for substance-use programs
- Provide support for medication access or psychiatry referrals
- Provide information on substance-use support groups or peer programs
The Houston-HVIP helps identify employment opportunities and support the skills and tools needed to obtain employment.
- Discuss employment goals, interests, and barriers
- Assist client in researching and/or contacting potential employers
- Assist client with preparing resume and completing job applications
- Complete mock interview and/or provide interview advice to client
- Refer to job training or workforce programs
- Assist with accessing professional clothing or grooming support
The Houston-HVIP provides information and connections to educational programs aligned with the client’s long-term academic and professional goals and interests.
- Provide informational resources about educational programs, including GED, trade schools, community colleges, and other college programs
- Assist client with determining eligibility and preparing application materials for educational programs
- Assist client in accessing tutoring and/or academic mentoring
- Help client develop study and time-management skills
The Houston-HVIP can assist eligible clients with helping to access transportation services.
- Assess client’s transportation needs and options
- Help apply for subsidized or discounted transit fare programs
- Provide information and educate on using public transportation or transit apps
The Houston-HVIP can provide information on mentorship and skill-building opportunities to aid in the recovery journey.
- Refer clients to mentorship or enrichment programs
- Assist in enrolling clients in GED, ESL, or life skills classes
- Assist in completing applications for leadership programs
- Assist client with finding mentorship or peer-support services
The Houston-HVIP connects clients with resources to support child care and provide positive parenting practices.
- Discuss child care needs and available options
- Refer client to child care subsidies or Head Start programs
- Refer eligible clients to parenting classes or family resource centers
- Help with locating after-school or summer programs
- Support in obtaining birth certificates or immunization records for enrollment
The Houston-HVIP assists clients with navigating the civil and criminal legal matters related to their violent injury.
- Assist with applying for legal aid assistance programs
- Assist in organizing paperwork related to family, housing, or benefits cases
- Support client in requesting ID restoration or expungement
- Provide client with emotional support through court date
- Assist with victim impact statement
The Houston-HVIP assists clients in developing safety plans and connecting with external services to support safety and well-being.
- Provide safety planning for client
- Refer eligible clients to intimate partner violence resources
- Support client in making a police report and navigating communication with law enforcement
- Assist client in completing a child protective services report
- Assist eligible clients in finding and applying for emergency housing programs
