The UTHealth Houston Homeless Outpatient Mental Health Expansion Services (UTHealth Houston HOMES) Program funded by the Substance Abuse and Mental Health Services Adminsitration (TIEH Grant)

The overarching goal of the UTHealth Houston HOMES program is to promote long-term mental health recovery, reduce substance use and ensure access to primary care, income supports, and permanent supportive housing for people experiencing homelessness with serious mental illness or co-occurring disorders.

Funded in 2018, UTHealth Houston HOMES has served over 500 people experiencing homelessness providing the following services: 

(1) Outpatient behavioral health treatment using evidence-based interventions:  Motivational Interviewing and Cognitive Behavioral Therapy

(2) Linkage to psychiatric medication management;

(3) Primary medical care and care coordination by a community health worker (CHW);

(4) SSI/SSDI Outreach, Access, and Recovery (SOAR) case management;

(5) Linkage to supported employment;

(6) Housing assessments and navigation through HUD’s Coordinated Access
