We accept most insurances and offer financial assistance plans for those who are uninsured and/or do not have adequate financial resources for necessary health service provided. Call to learn more or to make an appointment at 713-486-2630.
For your first appointment, please bring the following:
- Photo identification
- Insurance Card
- List of current medications and/or prescription bottles
- Medical records
- Discharge paperwork from any hospitalizations
- Psychological testing reports
- Name and contact information of all current healthcare providers
For subsequent appointments, please bring the following:
- Photo identification
- Insurance Card
- List of current medications and/or prescription bottles
- Recent paperwork regarding any labs, testing, and/or new providers you are seeing
