Training Verification
Overview
The Residency Training Office provides residents and fellows who have completed medical education training at the Louis A. Faillace, MD, Department of Psychiatry & Behavioral Sciences in McGovern Medical School, part of The University of Texas Health Science Center at Houston with training verifications. To offset the cost required for credentialing our current trainees, there is a cost associated with requesting a training verification. Please see below for more detailed information.
General Psychiatry Residency Program & Child and Adolescent Psychiatry Fellowship Program
All verification requests must provide an Authorization for Release of Information signed by the alum, instructions for the verification as well as payment. Cost is determined by the number of years since the alum graduated from his or her training program. Please see below for details.
For those who:
- Graduated 3 years or less – training verifications are completed at no charge.
- Graduated more than 3 years ago – training verifications are completed at the following price points:
- $75.00 – standard verification letter on university letterhead that includes the following details:
- Name of Trainee;
- Training Program and Specialty;
- Dates of Training;
- Confirmation of successful completion or explanation and further information, if not completed successfully; and
- Current contact information for residency program.
- $125.00 – for requests that require completed documentation beyond the standardized verification letter. These requests may include:
- Training Verification forms that require explanation letters and/or Program Director’s signature.
- $75.00 – standard verification letter on university letterhead that includes the following details:
Psychology Internship Program
Training verifications for graduates of the Psychology Internship Program are provided at no cost.

We accept payment made by check or money order only. We apologize for the inconvenience.
