Scheduling & Payment
We accept most insurances and offer financial assistance for those who are uninsured and/or do not have adequate financial resources for necessary health service provided. Call to learn more or to make an appointment at 713-486-2630.
For your child’s first appointment, please bring the following:
- Photo identification
- Insurance Card
- List of current medications and/or prescription bottles
- Medical records
- Discharge paperwork from any hospitalizations
- Psychological testing reports
- Name and contact information of all current healthcare providers
For your child’s subsequent appointments, please bring the following:
- Photo identification
- Insurance Card
- List of current medications and/or prescription bottles
- Recent paperwork regarding any labs, testing, and/or new providers you are seeing
Additional information:
- ARD meeting paperwork
- Report card
- School counseling notes
- Legal guardianship documentation
*Please note that only the legal guardian can bring a minor to the new patient appointment. In addition, if someone other than the legal guardian will be bringing a minor to a follow-up appointment, a consent to escort a minor must be completed at least 24 hours prior to the appointment. Please contact the clinic in advance to arrange.
