UTHealth Houston Neurosciences hosts the Stroke Transitions of Care and Post-Stroke Care Conference at the Cizik School of Nursing at UTHealth Houston, Oct. 6-7. The conferences is a dynamic, engaging, and inclusive meeting designed to bring together leaders in stroke care and research, patients with stroke and their caregivers, and community and industry stakeholders to establish the best practices and define priorities to improve stroke transitions of care and post-stroke care.
