Patient Care
We have developed parallel geriatric and palliative care services at LBJ General Hospital and Memorial Hermann Hospital in the Texas Medical Center. LBJ Hospital is part of the Harris Health System, one of the largest hospital districts in the United States. Memorial Hermann-Texas Medical Center is part of the Memorial Hermann, the largest not-for-profit health system in Southeast Texas. In addition, private outpatient clinics and a house call program are supported through UT Physicians and the Harris Health System.

Memorial Hermann Hospital – Texas Medical Center
Geriatric ACE (Acute Care for Elders) unit at Memorial Hermann Hospital in the Texas Medical Center. The ACE unit is managed by an interdisciplinary team that treats the acute issues of older patients requiring hospitalization, and focuses on prevention of risks that accompany hospitalization. The team works to assure a successful discharge that meets the needs of the patient and the family.
Geriatric Consults – The consult service cares for geriatric patients on other surgical and medical sub-specialty services.
Skilled Nursing Unit – The unit is dedicated to short-term skilled rehabilitation needs post-acute hospitalization.
Supportive and Palliative Care Consults – The Supportive Medicine Service is a consult service that provides palliative care to hospitalized patients at MHH TMC.

Geriatric Consult – Expert inpatient consultation services are available to assist with evaluating and treating Geriatric patients and addressing their needs.
Palliative Consult – Expert inpatient consultation services are available to assist with evaluating and treating patients with life limiting illness and providing support for patients and their families.
Palliative Outpatient – Outpatient clinic services are available to evaluate and treat patients with life limiting illnesses, provide aggressive symptom management and help patients and their families achieve and maintain a good quality of life.

House Call Program – In Homes and Assisted Living Centers – The Settegast Geriatrics Clinic has a House Call Program that provides medical care at the patient’s residence.

Baytown Community Health Center, 1602 Garth Road in Baytown, tel. 281-827-2700. An outpatient clinic for geriatric primary care. Located on Garth Road near the interchange of Highways 146/201 and 330.
Bayland Geriatric Health Center – 6400 Bissonnet St in Houston, tel. 713-873-6830. Offering seniors wellness checkups, geriatric assessments, social services and minor illness care in Southwest Houston.
UT Physicians
Geriatric House Call Program – In Homes and Assisted Living Centers – The Center for Healthy Aging at Bellaire has a House Call Program that provides medical care at the patient’s residence.
Center for Healthy Aging – 6500 West Loop South, Suite #200, Bellaire, TX 77401, tel. 713-486-5150. Located in the UT Physicians facility in Bellaire, the Center for Healthy Aging is a comprehensive outpatient clinic for geriatric primary and subspecialist care. Patients with memory problems or dementia may be seen in the Brain Health Clinic by an interdisciplinary team that includes a Geriatrician, Gerontological Nurse Practitioner, Neurologist, Geriatric Psychiatrist, Neuropsychologist, Nurse, and Geriatric Social Worker. The clinic space on the first floor is specially designed and accessible for elderly. Lab and imaging services are located on the same floor of the building.

Kindred Hospital, Houston Medical Center
Palliative Consult– Kindred Transitional Care Hospitals, certified by Medicare as long-term acute care hospitals and licensed as acute care hospitals, can provide support for patients needing palliative care.
