Prescription Refills:
Request at least 1 week before medication runs out.

Complete a release form (download at utphysicians.com) to obtain records.


Parking:
Parking in the parking garage behind the UT Physicians building is free. There is a drop off area as you enter the driveway from the 610 feeder road and handicapped parking is also available adjacent to the building. Handicap parking is available on the roof of the garage. Use the north entrance on 1st street behind the building.

Scheduling an Appointment

To schedule a clinic visit or house call, call 713-486-5150 during business hours. Please arrive 30 minutes early to complete paperwork. Initial evaluation usually takes at least 2 hours.
