Official Functions/Events

  1. Email all requests for Official Functions/Events to Telisha Green, ([email protected]), Sr. Executive Assistant to the Chairman. Please be sure to submit all items below for pre-approval 30 days in advance of your event.
    • Official Function Form (sample of form and guidelines attached) *Only highlighted fields should be completed* See Official Function Form here
    • Permission to Serve Alcohol Form (If needed) Please refer to HOOP 9 Policy regarding Alcoholic Beverages.
    • Agenda for the event
    • Supporting emails from the account manager for this request
    • Budget allowance (If this request is related to a grant, contract, or gift account, then budget justification or awarded budget is needed.)
    • CFS
  2. Approval Process – After review, Telisha will complete the following steps on your behalf:
    • Initial for pre-approval
    • Submit request to DMO for review and approval
    • Submit to the Dean’s Office and Risk Management for final review and approval.
    • Once final approval is acquired, Telisha will return all Official Function documentation to the requestor.
  3. Event & Payment *All remaining steps are the requestor’s responsibility*
    • Execute the event as planned.
    • Maintain and update the approved form, as appropriate.
    • Within 30 days of the event, email the approved form, supporting documentation, and invoices to [email protected] for payment
  4. Changes and Cancellations *If the date of your event changes or is cancelled, please notify Telisha Green immediately*
    • Updated official functions request with the new date will be needed
    • Updated permission to serve alcohol form with the new date is needed
    • Updated Agenda for the event
    • Updated supporting emails from the account manager for this request
    • Budget allowance (If this request is related to a grant, contract, or gift account, then budget justification or awarded budget is needed.)
    • CFS

ALL approvals must be received to proceed with the event as requested. Due to the COVID-related restrictions in place at this time, events are being considered on a case-by-case basis.
