Verification of Residency Training

The Department of Obstetrics, Gynecology & Reproductive Sciences Residency Program at McGovern Medical School at UTHealth Houston provides verification of residency training for physicians who have completed training within the program. Verification requests are processed for credentialing organizations, hospitals, licensing boards, employers, and other institutions requiring confirmation of training and professional standing.

Verification Fees:
Verification requests will be processed upon receipt of payment.

$75.00 – Standard verification letter on UTHealth Houston letterhead
$75.00 – Verification form completed by the Program Director (form provided by the requesting institution)
$150.00 – Both a verification letter and completed verification form

Requests for Training Completed More Than Five Years Ago
Verification requests for physicians who completed training more than five (5) years ago may require additional record retrieval and processing time. Payment must be received before records are retrieved and the verification process begins.
