Quality Improvement Proposal: Multidisciplinary Asthma Clinic for Patients with Uncontrolled Asthma

This quality improvement initiative, led by Dr. Ekta Perera, focuses on optimizing care for patients with uncontrolled asthma through a Multidisciplinary Asthma Clinic (MAC) model. The project includes mapping high-burden communities, implementing an eight-month prospective cohort receiving comprehensive asthma care, and evaluating outcomes against matched controls. The goal is to refine and expand the MAC model to improve asthma management and reduce disparities across diverse patient populations.
