Radiology Fellowship Verifications

All verification requests can be emailed to [email protected] and must include a Release of Information authorization signed by the alumnus or alumna, detailed verification instructions, and payment.

A $100 handling fee applies to each verification. Payment may be made by check or credit card.

To pay by check, please send a cashier’s check, money order or Institutional check made payable to the Department of Radiology – UTHSC, and mailed to:

Credit card payments are accepted at https://go.uth.edu/radiologyedverification (external link). Please complete all required fields to ensure prompt service.

Verification requests will not be processed until after receipt of the fee. If payment is not received within 60 days, the original request will be discarded.
