Faculty / Staff Recognized for Excellence at CS&E Graduation & Annual Quality Fair

Our team proudly celebrates the accomplishments of its trainees and faculty, highlighted by recent successes at the Clinical Safety & Effectiveness (CS&E) Graduation and the annual Quality Fair at Memorial Hermann Hospital – TMC.

Congratulations to EM graduates Benjamin Clark, MD, Tina Chacko, M.S., PA-C, and Vishal Patel, MD on reaching this important milestone. Their hard work and dedication shine through, and they embody our commitment to training the next generation of leaders in emergency medicine.

As graduates of the Clinical Safety and Effectiveness (CSE) program, a hands-on course that equips frontline clinicians with practical quality-improvement skills, they’ve not only grown as providers but also strengthened our shared mission to elevate patient care across the system.

The department also had a strong presence at this year’s Quality Fair, with Hospitalist faculty earning top recognition for their work in quality improvement and patient care innovation. The project titled “Outpatient Parenteral Antimicrobial Therapy (OPAT) Process Redesign” received 1st place, while “Surgical Time-Out: Early Payor Status Checks to Prevent Denials” earned 3rd place.

In addition, Jeff Chen, MD was recognized for his work highlighting how Memorial Hermann Texas Medical Center achieved a #1 ranking in the Vizient safety domain.

Further recognition came from the PEM team, which received first place for the Graham Award for their quality improvement initiative focused on pediatric sepsis. This award honors standout contributions to patient safety and quality improvement, especially innovative educational efforts within training programs.
