Research Conflict of Interest	Prior to all START submissions need to complete the Research Conflict of Interest (RCOI) in START after access has been granted.

Biosketches	All Principal Investigators, Co-Investigators, Investigators, and Key Personnel will need one. A template and sample can be found on this link: https://grants.nih.gov/grants/forms/biosketch.htm
Bibliography Link Creation	Please email Selwyn Chopin-Sanderson so Instructions can be sent to create this. Will need to be created on NCBI. https://www.ncbi.nlm.nih.gov/account/

For studies that involve Human Subjects, please complete the following below
Regulatory Compliance	Please provide most resent signed and dated Curriculum Vitae (CV) and a current Medical Licence to Trinette Polydore Trinette.L.Polydore@uth.tmc.edu . CV’s will need to be updated annually.
CITI Training	CITI Training will need to be completed. Please register at: https://www.citiprogram.org/index.cfm?pageID=154&icat=0&ac=0&region=1&message=0 Please be sure to select University of Texas Health Science Center at Houston when registering. Will need to complete Human Subjects Research-Biomedical and Good Clinical Practice (GCP) Modules.
