For All Submissions
Complete START Training to serve as PI	Must register for classes via iLearn. Follow this link https://uthealthhouston.sumtotal.host/rcore/c/dash/home/homenew?isDeepLink=1 (external link) Also, please complete the User Access Form Request found here:
Budget Template	Can be found on the UTSTART site. Link: https://www.uth.edu/sponsored-projects-administration/tools-resources/forms-templates (external link) The budget should also be accompanied by a Budget Justification please email us for an example.

Research Conflict of Interest	Prior to all START submissions need to complete the Research Conflict of Interest (RCOI) in START after access has been granted.
For studies that involve Human Subjects, please complete the following below
Regulatory Compliance	Please provide most resent signed and dated Curriculum Vitae (CV) and a current Medical Licence to Trinette Polydore [email protected] . CV’s will need to be updated annually.
CITI Training	CITI Training will need to be completed. Will need to complete Human Subjects Research-Biomedical and Good Clinical Practice (GCP) Modules.
