Integrated molecular and high-contrast anatomic imaging in a single examination
Overview
PET/MRI combines positron emission tomography (PET), which shows the biologic distribution of a radiopharmaceutical, with magnetic resonance imaging (MRI), which provides high-resolution anatomic and tissue-characterization information. The two datasets are registered and interpreted together, allowing molecular findings to be correlated directly with detailed MRI anatomy.
FDA Approval and Who May Benefit
PET/MRI is an FDA-regulated diagnostic imaging technology. The PET component uses an FDA-approved or otherwise appropriately authorized PET radiopharmaceutical for the clinical question, while the MRI component is tailored to the body region and indication. PET and MRI data may be acquired simultaneously or sequentially on an integrated system.
Why This Imaging Is Useful
PET/MRI can be especially valuable when superior soft-tissue contrast, multiparametric MRI, or reduced CT-related ionizing radiation is desirable. Depending on the radiotracer and clinical question, applications may include oncologic staging and restaging, brain imaging, selected neurologic indications, prostate cancer, neuroendocrine tumors, pelvic malignancies, and other complex cases where combining molecular and MRI information may improve characterization.
What to Expect / Typical Protocol
- Before the examination: The clinical indication and appropriate PET radiopharmaceutical are reviewed. Standard MRI safety screening is performed, including review of implanted devices, metallic foreign bodies, and other contraindications.
- Radiopharmaceutical administration: The selected PET tracer is administered according to its approved protocol, followed by the appropriate uptake period.
- Integrated acquisition: PET imaging is obtained with MRI-based attenuation correction. MRI sequences are tailored to the indication and may include dedicated diagnostic sequences of the brain, abdomen, pelvis, musculoskeletal system, or other regions.
- Contrast: Intravenous gadolinium-based MRI contrast may be used when clinically indicated and safe. The need for contrast is determined by the examination protocol and patient-specific factors.
- Interpretation: PET and MRI findings are reviewed together to provide an integrated assessment of molecular activity and anatomy. The final protocol may be customized after review of prior imaging and the specific clinical question.
