A comprehensive outpatient clinic specializing in assessing, diagnosing, and treating persons with memory disorders and/or dementia. The team includes geriatricians, neurologists, geriatric psychiatrists, neuropsychologists, gerontological nurse practitioners, and geriatric social workers.
- Comprehensive assessment of memory problems and/or dementia
- Additional testing and diagnostic studies, as indicated
- Neuropsychology testing
- Meeting with the patient and family to discuss the diagnosis and plan of care
- Ongoing support of the patient and family through counseling, resources, education, and follow-up
- Continuity of medical care

Contact the clinic at the number above at least one week before you run out of your medications. Triplicate prescriptions must be requested 7 days prior to pick up.
If you need a copy of your medical records, please complete a release form and we will forward it to Medical Records.
Parking in the parking garage behind the UT Physicians building is free. There is a drop off area as you enter the driveway from the 610 feeder road and handicapped parking is also available adjacent to the building.
