Epic – Harris Health System – Human Studies Research Authorization

All human studies research staff are required to complete an authorization application.

Please follow instructions on this form – Authorization Application – Initial and complete this form- Sponsoring Researcher Agreement-initial to receive research authorization at Harris Health System. There are additional items listed on the application as well as the attached documents that will need to be submitted. Please submit all documents in PDF format at one time. Only completed applications are accepted. Incomplete applications will be returned.

**It is imperative that the submitted forms are completed appropriately as the information on the forms are used to provide you with the access needed at Harris Health for your job role. Providing incorrect information will significantly delay the process.**

New Requirement Alert:
*All researchers are now required to be fully vaccinated against COVID-19.

Bilingual Qualification:
Harris Health now provides Research Staff the opportunity to become qualified as a bilingual workforce member. The qualification is a two (2) part process:  1) assessment and 2) oral examination. You are not required to take the assessment to become qualified as a bilingual workforce member.  However, if you decline the qualification, you will not be permitted to speak in any other language, other than English, with patients while doing your job.

Epic Access Level:
** Read-only research access: Appropriate for research users who do not need to enter data into the electronic medical record (Epic), document in the chart, or place orders.
** Research coordinator data-entry: Appropriate for research coordinators who need to enter data into the electronic medical record (Epic), document in the chart, or place orders. Intended for non-licensed (i.e., non-nurse, non-physician) coordinators.

Phlebotomy Check Off
Harris Health requires anyone drawing blood to complete a phlebotomy check off. The Phlebotomy Check off is performed by a Harris Health employee who has been designated as such. Once proof of training has been submitted you will be scheduled for your checkoff. This is a new requirement and will need to be fulfilled before receiving Epic access at Harris Health. This is a one-time requirement.

Important
Protocols listed on application must be active, i.e. with IRB and Harris Health System administrative approvals.

Once all required documents are submitted, turnaround time for approval is 14 business days.

**All incomplete application submissions are discarded after 30 days.
