How to complete the Donor Form and Special Request Form:

1. Download this file – Willed Body Program Donor Form (PDF updated Spring 2024) (NOTE: The download contains the Donor Form, the Special Request Form and some Frequently Asked Questions about the program.)
2. Print the forms out using your printer.
3. Fill out the Willed Body Program Donor form AND the Special Request Form.
4. Have two (2) witnesses sign the form. (Designate a NEXT OF KIN at the bottom of the first page.)
5. Mail the original forms to:
