How to complete the Donor Form and Special Request Form:

1. Request a copy of the Willed Body Program Donor Form (NOTE: The form contains the Donor Form, the Special Request Form and some Frequently Asked Questions about the program.)

2. Print the forms out using your printer.
3. Fill out the Willed Body Program Donor form AND the Special Request Form.
4. Have two (2) witnesses sign the form. (Designate a NEXT OF KIN at the bottom of the first page.)
