Care4 Credentialing Process For Memorial Hermann- Texas Medical Center
Research Credentialing is appropriate for researchers who need to complete non-interventional (no patient contact) research duties at Memorial Hermann. Non-interventional duties include, but are not limited to, accessing Memorial Hermann medical records, screening and consenting, and survey administration.
The research credentialing process consists of submitting the appropriate paperwork to Memorial
Hermann’s Clinical Innovation and Research Institute (CIRI) and appearing for an in-person verification interview with CIRI staff.
To begin the process, please submit the following documents to Joann Ivey,

Please submit the below required documents via email as separate attachments:
1.CV/Resume
2.The IRB approval letter for each study you have been added to as study personnel.
3.Job description
Note: Not an official UT job description. Please supply a list of all research-related duties you will perform in Memorial Hermann (e.g. accessing medical records, consenting, phlebotomy,
administration of study drug, etc.)
4.Research Credentialing Form (004)
Note: Please include the IRB HSC number(s), title(s) and PI name(s) of all studies you are involved with under question II and III.
5.HIPAA Agreement Form (005)
6.Electronic Medical Record Access Request Form (003)
7.Copy of COVID vaccine card
Once we receive the above completed documents, an in-person interview with a Clinical Research Specialist will be scheduled. The Clinical Research Specialist assigned to your file will be your primary contact throughout the credentialing process.
If your job duties in MHHS include interventional research duties such as phlebotomy or duties that require licensure (i.e. RN) you will be required to also complete an application for Allied Health Credentialing.
