You can go online to complete the application for financial assistance. Go to Patients, then Access Care. You can get an application in English, Spanish, or Vietnamese, and mail it to HCHD Financial Assistance Program, P.O. Box 300488, Houston, TX 77230. Or you can pick up and drop off applications at one of five Eligibility Center locations listed on the website. When you come in, you will need to have several forms of identification. You can call 713-526-4243 to speak to a scheduler who can give you information over the phone about how to apply for assistance.
